Membership of the HPA is open to all Healthcare Scientists working in Physical, Physiological and Enginereering Science. The membership subsciption is most conveniently paid by monthly Direct Debit although other payment methods are available. To join the HPA follow these instructions.

1. Download the application form and print off a copy
2. Fill in your details
3. Select your preferred job code and description from this list:

4. In the box marked “for union use only” enter:
   Branch No 0059 (this ensures you will join the HPA National Branch) and your preferred Job Code
5. Send the completed form to Unite (address on the form)

The 2009 Subscription rates are as follows: (from The Unite web site)

Full time	£120 pa, £11 pm
Part time	£55 pa, £5.50 pm
Under 18s	£12.96 pa, £1.08 pm
Trainees	£12.96 pa, £1.08 pm
